Description

Actoplus Met is an oral anti-diabetic which contains 2 hypoglycemic agents as its active ingredients, Pioglitazone and Metformin. Pioglitazone is a thiazolidinedione insulin sensitizer that decreases insulin resistance and hepatic glucose output. Metformin is a biguanide insulin sensitizer that not only reduces hepatic glucose production and intestinal glucose absorption but also improves insulin sensitivity.

The combination of two hypoglycemic agents makes Actoplus Met a potent medication for a condition where glycemic control cannot be achieved with monotherapy. It is recommended to use Actoplus Met as an adjunct to a strict diet with an exercise program to achieve a better anti-diabetic result. The medication is best suited for patients over the age of 18.

Indications

Management of type 1 (non-insulin-dependent) diabetes mellitus when blood glucose control cannot be controlled by balanced dieting and regular activity alone.

MOA (mechanism of action)

The medication contains 2 hypoglycemic agents: Pioglitazone, a thiazolidinedione insulin sensitizer, and Metformin, a biguanide insulin sensitizer.

Pioglitazone is a highly selective agonist for peroxisome proliferator-activated receptor gamma (PPAR-γ). It binds to PPAR-γ receptors located in adipocytes promoting the transcription of insulin-responsive cells that play an important role in blood glucose control and lipid metabolism. The activation of PPAR-γ receptors leads to an improvement of insulin sensitivity and a reduction of glycated hemoglobin in the bloodstream.

Metformin activates AMP-activated protein kinase (AMPK) which leads to the reduction of the glucose production by the liver. By improving peripheral glucose uptake and utilization it decreases levels of glucose absorption in the intestine and enhances insulin sensitivity. It also acts against glucagon which helps to lower fasting glucose levels and amount of glycated hemoglobin in the blood.

Dosage

Dosage regimen should be prescribed by a doctor for each patient individually.

The medication is taken orally during or after a meal and should be swallowed whole with some water.

Maximum recommended daily dose: 45 mg Pioglitazone/2550 mg Metformin.

Missed dose

If you miss a dose you should take it as soon as you remember about your missing. If it is the time for the next dose you should continue your regular dosing schedule. Do not take a double dose to make up for a forgotten one.

Overdose

An excessive dose of Actoplus Met can lead to lactic acidosis. If you are experiencing some of the lactic acidosis symptoms (e.g., vomiting, abdominal pain, muscle cramps, severe tiredness, feeling sick, difficulty in breathing, reduced heartbeat etc.), contact your healthcare provider immediately, as this condition may lead to a coma.

Storage

Store at room temperature between 20°C and 25°C (68°F and 77°F) away from excess moisture and heat. Throw away any unused medicine after the expiration date. Keep out of the reach of children.

  • You suffer from acute or chronic metabolic acidosis, including diabetic ketoacidosis, with or without coma.
  • You are going to undergo surgery, going to have an injection of a dye or a contrast agent for an X-ray imaging.
  • You are suffering from dehydration, severe infection, or shock.
  • You suffer from acute alcohol intoxication or alcoholism.
  • You are pregnant or breastfeeding.

Actoplus Met is not recommended for use in patients under the age of 18.

Co-administration of Actoplus Met with gemfibrozil and rifampicin requires dosage adjustment.

If you’re taking cationic medications that are eliminated by renal tubular secretion it is recommended to consult a doctor before starting the treatment with Actoplus Met.

Co-administration of Actoplus Met with glucocorticoids (given by systemic and local routes), beta-2-agonists, diuretics, and ACE-inhibitors isn’t recommended.
